Oral GLP-1 Side Effects: What to Expect and What to Do About Each
Oral GLP-1 side effects are common, mostly manageable, and typically improve over time. Here's what patients most often experience and what actually helps for each.
Nausea
The most common oral GLP-1 side effect. Manifests as feeling queasy, particularly after larger meals or high-fat foods. Rarely produces actual vomiting for most patients.
What helps: eat smaller portions, avoid high-fat and greasy foods when nausea is present, sip fluids between meals rather than with meals, and let meals last longer rather than eating quickly. Ginger, peppermint tea, and simple bland foods can help acute episodes. Nausea typically improves substantially after the first 4-6 weeks.
Constipation
Slowed gastric emptying and reduced food intake can produce constipation. Manifests as less frequent bowel movements, harder stools, or difficulty passing stools.
What helps: increase water intake (aim for at least 2-3 liters daily); increase fiber intake through vegetables, fruits, and whole grains; regular physical activity; and over-the-counter stool softeners or mild laxatives if needed. Diarrhea
Less common than constipation but does occur. Usually mild and self-limiting.
What helps: staying well-hydrated (particularly with electrolyte-containing fluids), avoiding food triggers (spicy or very high-fiber foods that seem to worsen it), and probiotics or plain yogurt. If diarrhea is severe or persistent, consult your prescriber.
Fatigue
Some patients experience fatigue during titration, possibly related to reduced caloric intake or the body adjusting to metabolic changes. Usually resolves within a few weeks.

Reflux and Heartburn
Slowed gastric emptying can worsen reflux in patients prone to it. Manifests as burning in the chest or throat, particularly after eating or when lying down.
What helps: smaller meals, avoiding late-evening eating, elevating the head of the bed, and over-the-counter antacids. Consult your prescriber if reflux is significant — it may warrant a proton pump inhibitor or other treatment.
Injection Site Reactions (Injectable Only)

Any side effect that's severe, persistent beyond a week or two, or that concerns you warrants prescriber contact.
